First Edition! Henri Francois Le Dran's (1685-1770) "important discussion on cancer for the first time discarded the humoral conception of the disease. He regarded cancer as a local disease in its early stage and knew that it spread via the lymphatics to regional nodes, and from there into the general circulation. He described with great clarity the path of metastasis in breast carcinoma, including involvements of the lungs." Garrison & Morton "Als eine der umfassendsten Arbeiten aus dieser Zeit, in der sowohl die Ansichten der angesehensten Mitglieder der königl. Akademie zur Geltung kamen, als auch eigene Beobachtung und scharfsinnige Kritik in besonders auffallender Weise sich bemerkbar machten, ist die in den Memoires de l'Academie Royale de Chirurgie ) im Jahre 1757 veröffentlichte Zusammenstellung von Henri Franpois Le Dran ) zu betrachten. Obwohl im großen und ganzen ein Anhänger der Lymptheorie des Krebses, weichen seine Ansichten doch so vielfach von den seiner Zeit herrschenden ab, daß wir uns etwas genauer mit dem Inhalt dieses Artikels beschäftigen müssen. Die Arbeit ist in 4 Abschnitte eingeteilt: 1. Cancer der Haut, 2. Cancer der Mamma, 3. Cancer, "qui se forme par le reflux des évacuations menstruelles", zur Zeit der Menopause und 4. des Cancers "produits par le vice de la lymphe". Le Dran ist einer der ersten, der auf den lokalen Beginn des Krebses, besonders bei dem Haut- und Drüsenkrebs aufmerksam machte. "Le Cancer commence par l'engorgement d'un ou de plusieurs grains glanduleux, qui peu à peu se transforment en une substance compacte et dure, laquelle cependant est pénétrée par les liqueurs, qui y sont apportées par les vaisseaux." Nachdem Le Dran beim Haut- resp. Gesichtskrebs, den er gleichfalls als "Noli me tangere" bezeichnet, die Ursachen (Temperatureinflüsse, Irritationen usw.) der Entstehung geschildert hatte, suchte er durch die Therapie nachzuweisen, daß diese Art des Krebses nur lokal sein kann, da er durch gute Kaustika vollständig heilbar sei. Er operierte gut und ausgiebig, z. B. Unterlippenkarzinome, und zwar mit glücklichem Erfolg. Allein, sagte er, es gibt auch Krebse "par un vice intérieur". "Cela ne se connoît presque toujours que par le retour de la maladie, qui reparoît en un autre endroit que celui qui avoit été attaqué le premier", und diese Fälle wären für die Therapie aussichtslos. Das Mammakarzinom entsteht, nach Le Dran, auch meistens durch ein "vice intérieur", und obwohl er, selbst im vorgeschrittenen Stadium, die Operation und Ausräumung der Achselhöhle vornahm, erlebte er doch so viel Rezidive, daß er in operativer Beziehung schließlich sehr pessimistisch wurde. Die Schwellung der Lymphdrüsen in der Achselhöhle, und die von diesen ausgehenden Rezidive erklärte er sich folgendermaßen: "Je sais, que la lymphe viciée peut s'y être arrêtée sans repasser dans le sang"; aber, wenn unglücklicherweise ein Tropfen von dieser fehlerhaften Lymphe in die Körperflüssigkeit gelangt, dann kann die ganze Lymphe verdorben werden. In der 3. Gruppe von Krebsen "de cause interne", behandelt er besonders den Uteruskrebs, bei dessen Entstehung ein Dérangement der Menstruation ein sehr wichtiger Faktor sei (Dabei ist rühmend hervorzuheben, daß Le Dran seine Patientinnen gynäkologisch selbst untersuchte und nicht durch eine Hebamme untersuchen ließ, wie es z.B. vielfach bei deutschen Ärzten der Fall war, die sich von dieser Frau erst über Beschaffenheit und Lage des Uterus berichten ließen und danach ihre Anordnungen trafen). Über die Ätiologie der 4. Gruppe, weshalb die "Liqueurs" an einer Stelle stocken, ist Le Dran nicht in der Lage, Aufklärung zu geben. Le Dran hat auch viele Sektionen gemacht und dabei Karzinome des Magens, Rektums usw. beobachtet, zog aber diese .

First edition of Le Dran s (1685 1770) Traité" containing chapters on inflammation, abscess, gangrene, sutures, hernia, dropsy, calculus, cancer, resection and amputation, aneurisms, fistulas, whitlow, etc. One of the foremost French surgeons and on of Haller s teachers, Le Dran was noted for his judicious observations and valuable case reports in military and general surgery. He was chief surgeon of the French Army in the 1730ies, was innovative in his approach to cancer and a skilled lithotomist who made improvements to the operation.
