In day-to-day living, a person obtains from those around him - those who make up his social world - what R. S. Weiss has called "the provisions of social relationships". The authors set out to determine the effect on mental health if people are lacking in these provisions. They have carried out a careful field study into the possible link between neurosis and the lack of social support. A necessary step was the construction of an instrument to examine systematically an individual's immediate social environment (his personal network). This is the Interview Schedule for Social Interaction (ISSI), which yields indices for what relationships a person reports he has available, and the extent to which he perceives these relationships as adequate for his requirements. In both method and findings, what has been achieved is a substantial contribution to psychiatric epidemiology. Quite contrary to what the authors expected, they have found no evidence that the lack of social support is causally related to the onset of neurotic symptoms. Instead, what predicts symptom onset is not the reported availability of social relationships, but how adequate the individual perceives them to be when he is facing adversity. The evidence points to the importance of the individual's personality and constitution. The implications of these findings may prove to be profound. The current social environment has probably to be very abnormal indeed before it contributes appreciably to the onset of neurosis. It is those people who are habitually dissatisfied with their environment and personal relationships who are vulnerable.
