Synopsis

The book includes process plant accidents occurring since publication of the 1988 edition, for example, the Phillips 66 Company Houston Chemical Complex explosion and fire, and the Piper Alpha disaster. The book is also concerned with the immediate technical causes of these diasaters and the changes in design and procedures needed to prevent them from happening again. Trevor Kletz is the author of "Lessons from Disaster: How Organizations Have No Memory" and "Accidents Recur".
