As the government stated, the aim is to employ 3000 case managers by 2008. These include practitioners not only from nursing but occupational therapists, physiotherapists and social workers. This book addresses the areas of defining the role of case manager/community matron, supporting people with long-term conditions, defining protocols and guidelines, reviewing dilemmas and solutions with engaging patients in participating with their care and pushing and supporting the roles. It includes reflections on case studies that will assist other case managers to make decisions in relation to care.
