This book is written as a companion guide to Cases and Concepts for the new MRCGP. The aim is help readers pull together the knowledge and communication skills required for success in the consultation components of the new MRCGP exam, namely Clinical Skills Assessment (CSA) and Clinical Observation Tool (COT). The book does this in three ways: by demonstrating some of the required consultation skills on the accompanying DVD, which features 13 typical consultations by identifying and teaching these skills in a further 20 case write-ups - these offer readers an ordered, step-wise approach to data gathering, clinical management and interpersonal skills by giving candidates 15 'mock cases' on which to practise, either alone or in small groups - these cases cover paediatric and telephone consultations, dealing with lists, breaking bad news and home visits. The book and DVD are meant to be interactive, with some of the DVD cases being accompanied by a CSA marking schedule for readers to assess the candidate' s performance. Readers are asked if the doctor on the DVD: asked the right questions, at the right time, in the right way performed the right examination correctly communicated in a precise, understandable and sensitive manner. This book has three distinct sections comprising 13 video cases, 20 long cases and 15 practice cases. For the section with video cases the reader is encouraged to read the scenario in advance and is given specific tasks looking at consultation skills. After watching the video there are review notes which aid critical appraisal of the consultation, suggesting alternative or further actions. The debrief section encourages the reader to think more deeply about various issued and consider how they might approach similar consultations. There are useful links to further reading. This is a great section to go through either alone or as a study group. It might be useful to include a wider variety of clinicians to appraise slightly different styles of consulting. The wide range of long cases could be used by groups or with trainers. The case summaries for doctor and patient are comprehensive. Evaluation is encouraged using the COT format, thus focussing on consultation skills useful for both formal COTs and ultimately the CSA. Structure and tips include links to guidelines. Evaluation of the practice cases using the CSA marking scheme is invaluable, including both positive and negative indicators. Together with the debrief notes, this should stimulate discussion and feedback after watching the consultation, for which extensive patient and medical notes are provided. The comprehensive marking guide is really beneficial to assist consulting styles. Overall this is a well written study guide incorporating a variety of ways to look at consultation skills, all of which could be used alone or with a group. The additional notes with all the cases help to focus on specific skills and target further revision. I really like the style of the book and presentation of the cases. Unlike other CSA books currently on the market, this book features a DVD containing 13 consultations, accompanied by detailed written commentary. By slowly working through the DVD cases with the accompanying reading, candidates learn the consultation skills being tested in COT and CSA. Cases 8 - 13 have a CSA marking schedule and invite candidates to mark the CSA case, thereby familiarising them with the consulting behaviours examiners look for. On completion of the 1st DVD section, candidates are presented with 20 mock cases on which to practise, either alone or in small groups. All 48 cases featured in the book are presented in the same format: namely, a brief to the doctor about the patient; a patient summary sheet; tasks the doctor should concentrate on in this case (that is, consultation skills this case emphasises); a brief to the patient (so the person playing the patient has the necessary information); a detailed analysis of how the doctor should have consulted (according to the CSA domains); a debrief; a test your theoretical knowledge section featuring true/false questions and signposting to the relevant literature.
