Le rétrécissement/obstruction pharyngée pendant le sommeil est souvent décrit comme un phénomène inspiratoire résultant d'une hypopnée obstructive/apnée. On ne sait cependant pas si l'hypopnée centrale peut entraîner un rétrécissement des voies respiratoires. Ce livre passe en revue des études originales récentes qui ont évalué les changements dans la mécanique des voies respiratoires supérieures et le calibre pendant le sommeil en utilisant des techniques d'imagerie. Les principales conclusions de ce livre sont que les voies respiratoires supérieures sont plus étroites pendant l'hypopnée centrale principalement pendant la phase expiratoire. L'inhibition de la puissance du moteur ventilé pendant le sommeil augmente l'observance palatine rétro expiratoire. L'augmentation de la résistance des voies respiratoires, la réduction du calibre pharyngé et l'augmentation de la conformité pharyngée en raison de l'hypocapnie, indiquent son rôle possible dans la pathogenèse de l'obstruction des voies respiratoires supérieures pendant le sommeil.
