Ce travail étudie l'idée d'un service de coordination qui améliore le flux d'informations entre les parties prenantes indépendantes le long du parcours de soins aux patients. Sur la base des exigences identifiées en matière de processus, de structure des TIC et de contraintes juridiques, un service de coordination spécifique aux accidents vasculaires cérébraux a été développé, validé, mis en œuvre et évalué en conséquence. Le service de coordination, appelé Stroke Manager, a été analysé en évaluant son efficacité, son efficacité et son acceptation dans un essai contrôlé randomisé de 18 mois dans la région allemande Rhön Grabfeld. L'essai contrôlé randomisé a confirmé un impact positif significatif sur la qualité de vie et les compétences des patients liés à la santé (HRQoL) tout en offrant un rapport coût-bénéfice positif et une acceptation par les parties prenantes concernées.
