Synopsis

Two adult patients are imaged on the same equipment, by the same technologist, within an hour. Both measure twenty-four centimeters front to back, so the technique chart gives the same answer for both.

One has an abdomen distended by gas, the other the same space filled by fluid, and the correct techniques differ by a factor of about four and a half. Nothing the chart measures can tell them apart.

This handbook closes that gap. Technique charts are built for the tissue they expect, and disease changes the tissue: it replaces air with fluid, strips mineral from bone, destroys alveolar walls, and introduces gas where there should be none. A technologist trained only on memorized charts discovers each change after the exposure, in a rejected image that cost dose and answered nothing.

The book teaches the mechanism instead. Each imaging modality measures one physical property, disease becomes visible only when it changes that property, and every console decision follows from knowing which property is measured and what the disease is doing to it.

From that one idea come the classification of pathology as additive, destructive, or attenuation-neutral, a quantitative exposure adjustment framework, and the recognition that an attenuation-neutral condition calls for a different modality, not a heavier exposure.

Working through its twenty-one chapters, you will learn to:

  • Classify any condition from its mechanism rather than a memorized list, deciding the direction of the technique response before its size.
  • Derive exposure adjustments with arithmetic short enough to run at the console, demonstrated step by step in forty-three worked examples.
  • Read ninety-seven fielded condition cards spanning chest, cardiac, skeletal, abdominal, urinary, pelvic, and neurologic disease, each with the same seven fields from mechanism to principal pitfall.
  • Recognize automatic exposure control failures, tangled distance, grid, and habitus corrections, and the conditions no exposure increase will ever reveal.
  • Describe findings in neutral, defensible language, escalate critical appearances through the correct pathway, and document within the professional scope boundary.
  • Manage dose deliberately, from each added projection to departmental repeat rates.

One hundred twenty-six practice problems close the chapters, every one solved in full in the answer key with the reasoning visible. Five appendices hold the equation reference, the exposure adjustment quick reference, the modality comparison matrix, reference values, and an international units and terminology map, with a glossary and subject index behind them.

The scope boundary is drawn on every chapter: the radiologist interprets; the technologist observes, describes neutrally, adjusts technique, escalates, and documents. This book makes that contribution larger, faster, and easier to defend.

Written for working radiologic technologists, radiography students facing a pathology course, educators, and imaging departments that want a common reference by the console.
