In 2024, first-time candidates educated in the United States passed the NCLEX-RN at 91.2%. First-time internationally educated candidates passed at 53.8%. Those are NCSBN’s own published figures — a gap of 37.4 percentage points, between two groups who both completed a nursing program and both had their education verified as comparable before they sat.
If experienced nurses fail at that rate, the examination is not only measuring whether you know nursing. It is measuring something else as well, and that something else is learnable. This short book is about that something else.
Four chapters, written for one reader

Every one is a decision where the habit you trained with produces a confident wrong answer — delegating a judgment, carrying out a prescription that is not safe, overriding a refusal, letting a relative interpret, reading a number in the wrong unit. Question on one page, the full explanation on the next, so you never see the answer while you are still deciding.
Each case is tagged with which of the four differences it is testing, so a miss tells you which chapter to reread. Each explanation gives the priority clue, what the item is really testing, the safety issue, a separate reason every wrong option is wrong, what would change the answer, and a one-line rule to carry into the next item.
Checked, not remembered
Every drug-name pair was looked up in DailyMed, the National Library of Medicine’s index of FDA-approved labeling, and printed only where the US name dominates. One pair that appears in almost every such list was removed, because the US drug returns no current labeling.
What this book is not
It is not a question bank and it does not pretend to be one. It is short on purpose: 24 cases that change how you read an item, not a thousand that let you practise the same mistake. If you want volume, the companion NCLEX-RN Exam Prep & Clinical Judgment carries 150 further cases and is sold separately.
And it does not suggest that nursing education outside the United States is worse. Nothing in the published statistics says so, and neither does this book.
