This book records the results of ten years' evaluative research carried out in the Medical Research Council's Social Psychiatry Unit at the Institute of Psychiatry, London. A psychiatric case-register was set up in 1964, in anticipation of a number of organizational changes that were intended to transform a traditional pattern of psychiatric care based on an out-of-town mental hospital, into a comprehensive and integrated community service. The joint Maudsley and Bethlem Royal Hospital took responsibility for the area, a new district treatment unit was opened, a rehabilitation workshop was set up, local authority social workers were given joint appointments with the hospital, day-care was expanded, and a sheltered housing association formed. These developments are described by the clinicians involved and illustrated by statistics from the Camberwell Register. The results of more intensive studies, based on samples derived from the Register, are also described. For example, the reasons for admission to hospital or for staying as long as six months as an in-patient were investigated. Other projects were concerned with questions such as: how many patients could be referred for psychotherapy under the National Health Service; what happens to patients who lose contact with the outpatient department; what is the value of occupational rehabilitation of chronically handicapped psychotic patients in the community; how do services for the mentally retarded compare with those for the mentally ill? Finally, there is a chapter on the Camberwell Reception Centre; an ever-present reminder of what may be the fate of those psychiatrically handicapped people who slip through the net of the community services. These results, together with a clinical and administrative review, are considered as a basis for planning a modern psychiatric service for a local community. Parts at least of the plan are already being put into action and themselves made the subject of further evaluation. The lessons to be learned are relevant not only to Camberwell but nationally and the book as a whole is a model of how evaluation and planning can work together in the service of patients and their relatives.
