L'édition originale a paru à Londres en 1777 sous le titre « First lines of the practice of physic ». Cullen introduit dans la terminologie médicale le terme de neurose, qui regroupe toutes les affections du sentiment ou du mouvement, sans fièvre primitive ou sans affection locale des organes (T.2, p. 185). Sa division des névroses en quatre ordres inspira Pinel. Il prône l'usage de la camisole (chemise étroite) pour contenir les fous furieux. Par ailleurs, il expose sa conception du diabète, en distinguant le diabète sucré et le diabète insipide. Il isole de la goutte le rhumatisme, subdivisé en rhumatisme aigu et chronique. William Cullen (1710-1790), né à Hamilton (Royaume Uni), devint médecin à Glasgow, où il obtint la chaire de médecine. Il devint ensuite professeur de chimie à Edimbourg, et enfin professeur de médecine clinique à l'Infirmerie Royale. Il est considéré comme l'un des plus grands médecins anglais du XVIIIe siècle. Cf.
