Dans le traitement des fractures, Malgaigne reconnaît le danger de l'immobilisation trop prolongée, et conseille les frictions. Ce traité comporte la description de la double fracture verticale du bassin, dite fracture de Malgaigne (T.I, pp.650 et suiv.). Malgaigne fut le premier à concevoir et à appliquer une méthode pratique de fixation externe, qu'il utilisa pour les fractures de la rotule (T.I, pp.771-775).
